Referrers
For professionals
Our experienced team is committed to providing your patients with the very best care. Here's how to refer.
How to refer
We prefer to receive referrals and correspondence electronically. Referrals do not need to be addressed to a specific doctor in our practice — diagnostic test referrals can be addressed to the practice or an unspecified doctor.
- HealthLink (preferred)
- EDI: cardiovs
- Fax
- (08) 8362 2273
- cvc@adelaide.edu.au
- Post
- 62 Beulah Road, Norwood SA 5067
Referrals are generally valid for 12 months from GPs and 3 months from specialists. We accept indefinite GP referrals for ongoing conditions, but Medicare requires a new referral for a new presenting problem, or if the referring doctor changes location or ceases practising.
Checklist for valid referrals
Patients cannot claim Medicare rebates without a valid referral at the time of appointment. A valid referral must contain:
- The patient’s name and date of birth
- Date of the referral
- Relevant clinical information about the condition for investigation, opinion, treatment and management
- Signature of the referring practitioner
